29 CFR  1926.451(a)(1):Each scaffold and/or scaffold component was not capable of supporting, without failure, its own weight and at least 4 times the maximum intended load applied or transmitted to it:   a)   YORKTOWNE HOTEL RESTORATION PROJECT, SOUTHEAST CORNER OF STRUCTURE - Two employees performed brick restoration services, from a Bennu High Speed Power Unit hydro-mobile scaffold up to ten stories above the grade.  The scaffolding system exhibited operational faults, including an inability to descend the climbing masts, which resulted in the scaffold's working surface experiencing a rapid, uncontrolled decent of approximately six stories, on or about August 31, 2021.  Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
