1926.20(b)(2) The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s):  a) Roof at 1721 Yost Road Blue Bell, PA- On or about September 3, 2021, employees were performing  were working from a low-slope rooftop location retrieving insulation material from a rough terrain forklift and were exposed to fall hazards of 15 feet to the ground below. An inspection was not initiated or conducted by a component person prior to the work activity.
