29 CFR  1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c):  (a)AB Contracting, LLC at the Brandon Redwood Water Tower Project located on South Chestnut Boulevard (483rd Avenue) in Brandon, SD 57005: On or about and at times prior to August 19, 2021, the employer did not ensure employees were protected from cave-in hazards. An employee performed work near the fire hydrant installation on the west side of 483rd Avenue installing 16 inch water pipe in a trench excavation approximately 8 feet in depth without adequate protective systems in place. The excavation was approximately 4 feet wide and had vertical walls. This condition exposed one employee to cave-in hazards.   (b)AB Contracting, LLC at the Brandon Redwood Water Tower Project located on South Chestnut Boulevard (483rd Avenue) in Brandon, SD 57005: On or about and at times prior to August 24, 2021, the employer did not ensure employees were protected from cave-in hazards. Employees manually packed fill around a water pipe and installed sheets of plywood to protect an unconnected pipe in an excavation with approximately 8-foot walls near the fire hydrant installation on the east side of 483rd Avenue without an adequate protective system in place. Employees performed work in close proximity to vertical walls within an excavation approximately 8 feet in depth. This condition exposed employees to cave-in hazards.   (c)AB Contracting, LLC at the Brandon Redwood Water Tower Project located on South Chestnut Boulevard (483rd Avenue) in Brandon, SD 57005: On or about and at times prior to August 26, 2021, the employer did not ensure employees were protected from cave-in hazards.  Employees performed work installing 16-inch water main on the east side of 483rd Avenue in a trench excavation approximately 10 feet in depth without adequate protective systems in place. An 8-foot high x 16-foot long trench box was located in the trench with approximately 2 feet of the excavation side wall above the top of the trench box. This condition exposed employees to cave-in hazards.   (d)AB Contracting, LLC at the Brandon Redwood Water Tower Project located on South Chestnut Boulevard (483rd Avenue) in Brandon, SD 57005: On or about and at times prior to September 8, 2021, the employer did not ensure employees were protected from cave-in hazards. Employees performed a tie-in of new pipe into an existing water pipe in an excavation approximately 8 feet in depth without adequate protective systems in place. The slope of the sidewall near a ladder used by employees to access the bottom of the excavation was approximated to be 57 degrees. This condition exposed employees to cave-in hazards.
