29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: Barracks Bldg. 5697 Exterior Spider Box 125377: Employee(s) used the spider box for temporary power to power tools and equipment. The black 50 Foot 50A 250V single-phase spider box cord end connected to the spider box was found with the strain relief pulled exposing the inner wires. Employees were exposed to electrical shock hazard(s)

The yellow temporary lighting stringer used to illuminate the utility hallway had cuts in the outer insulation exposing the inner copper wiring on the plug side. Employee(s) were exposed to electrical shock hazard(s).
