29 CFR  1910.134(k)(6): The employer did not provide the basic advisory information on respirators, as presented in Appendix D of 29 CFR 1910.134, in written or oral format to employees who wear respirators when such use was not required by the employer:   a)  McCreery & Sun of Colorado, Inc., at 1700 18th Ave in Greeley, CO 80631:  On or about August 23, 2021, and at times prior,  the employer did not provide Appendix D of 29 CFR 1910.134 to employees who wear air purifying respirators for voluntary use.  This condition exposed employees to respiratory hazards in that employees were not provided with training or information  for the proper use and limitations of air purifying respirators.  At least two employees attempted to use air purifying respirators to control exposure to phosgene gas.  Phosgene gas requires the use of supplied air respirators.
