29 CFR  1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries:  2631 Metro Boulevard: Employer is exposing employees to hazardous conditions where the employees work in areas where there are unmounted portable fire extinguishers. On and about 09/08/202, employees were exposed to a tripping hazard and a falling hazard where there were unmounted portable fire extinguishers.  Instances of the violation are:                                         01.  Shelving System, Row Number 10 where portable fire extinguisher was left on workplace floor.                                         02.  Warehouse where a portable fire extinguisher was left unmounted on a shelf.

The employees are permitted to work where permanent aisles are blocked.   On and about 09/08/2021, employees worked in the warehouse where a forklift was used.  Permanent aisles were blocked preventing the use of unobstructed aisles and passageways.

On and about 09/08/2021, employees were exposed to overhead falling object hazards where a sign warning of limit clearance was not displayed.  Employees worked where products were stored inches from overhead trusses.
