(a) American 97 Ton -  D/B Beaver (Barge/Crane) - On or about September 8, 2021 and at times prior thereto, employees were exposed to deep cuts and lacerations while operating and working near or around the unguarded alternator fan blades of the diesel engine used to raise or lower the spud barge's "donkey".
