29 CFR 1904.39(a)(1): Within eight (8) hours after the death of an employee as a result of a work-related incident, the employer did not report the fatality to the Occupational Safety and Health Administration (OSHA), U.S. Department of Labor: a) Southwest Airlines Co. located at 7050 Friendship Rd, BWI Airport, Baltimore, MD 21240: On or about August 23, 2021, an employee died of COVID-19. The employer did not report the work-related fatality to OSHA within 8 hours. The employer reported the death to OSHA on August 26, 2021.
