29 CFR 1910.212(a)(3)(ii):Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): JOB SITE: 41 St. Jude Industrial, New Madrid, MO 63866, on or about 8/31/2021, employees were permitted to operate a Roper Whitney Sheet Metal Press Brake, Model: 1012A, S/N: 685-08-087, 12G, without any form of guards to prevent employees from having any part of their body in the danger zone. As a result of the process, an employee sustained an amputation injury.
