29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch: a) 6 inch DeWalt Bench Grinder: On or about September 9, 2021 and times prior, an employee used a DeWalt 8" bench grinder to clean up square metal. The adjustable work rest was not adequately adjusted on the right side resulting in a 1/4 inch opening between the abrasive wheel and the work rest.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch: a) Maintenance Area: On or about September 9, 2021 and times prior, an employee used a DeWalt 6 inch abrasive bench grinder to clean up square stock metal. The grinder did not have the adjustable tongue guard on the right side adequately adjusted resulting in a 1/2 inch opening between the abrasive wheel and the top of the grinder.

29 CFR 1910.29(f)(4): Openings in stair rail systems exceeded 19 inches (48 cm) at its least dimension: a) Core Machine: On or about September 9, 2021 and times prior, employees used the stairway to access the core machine. The stair rail system was not equipped with a mid rail resulting in an opening of approximately 36 inches between the stair and the bottom of the hand rail.
