29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Maintenance Shop Rockwell Drill Press: On or about June 9, 2021 and times prior, employees used a Jet Drill Press to drill holes in parts. The rotating drill bit and chuck were not provided with a guard to prevent employee exposure to the flying chips and rotating parts. b) Machine Shop Dayton Drill Press: On or about September 2, 2021 and times prior, employees used a Dayton Drill Press to drill holes in parts. The rotating drill bit and chuck were not provided with a guard to prevent employee exposure to the flying chips and rotating parts.

29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch: a) 8 inch Delta Bench Grinder Left side: On or about September 2, 2021 and times prior, an employee used a Delta 8" bench grinder to sharpen tools. The adjustable work rest was not adequately adjusted on the Left side resulting in a 1/2 inch opening between the abrasive wheel and the work rest. b) 8 inch Delta Bench Grinder Right side: On or about September 2, 2021 and times prior, an employee used a Delta 8" bench grinder to sharpen tools. The adjustable work rest was not adequately adjusted on the right side resulting in a 1 inch opening between the abrasive wheel and the work rest.
