29 CFR  1910.178(l)(2)(ii): The employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace:  (a) Facility, 1050 Ohio Avenue, Glassport, PA:  On or about September 9, 2021 several employees who operate the powered industrial trucks were not provided with formal instruction which included lecture, discussion, video tape, computer learning or written material.

29 CFR  1910.178(l)(2)(iii): All operator training and evaluation was not conducted by persons who have the knowledge, training, and experience to train powered industrial truck operators and evaluate their competence:  (a) Facility, 1050 Ohio Avenue, Glassport, PA:  On or about September 9, 2021 the employer utilized a senior industrial truck operator to perform training of other operators and the person did not have the knowledge and training to conduct and evaluate their competence.  The "trainer" had not received any "train-the-trainer" instruction nor had the employee received any formal instruction by the employer.   The employee used as the trainer instructed the employees on the various controls on the equipment and what the job tasks were.  The training did not included detailed instruction on the vehicle lifting capacities, center of gravity issues, traveling, etc.

29 CFR  1910.178(l)(3)(ii)(I): Powered industrial truck operators did not receive initial training in other unique or potentially hazardous conditions of the workplace that could affect safe operation:  (a) Jobsite, 250 Summit Park Drive, Pittsburgh, PA:  On or before September 7, 2021 an employee was sent out with a driver to operate the powered industrial truck and load bundles of cardboard onto the truck.  The employee had not received training on the hazards associated with this task.  The employee was to load bundles of cardboard onto the truck and then ride on the elevator lift of the truck, while positioned in the powered industrial truck, and then push the stacks into position.   The employee had not been trained on this technique and when attempting to move the load forward lost traction on the ramp and drifted  backwards and fell off the back of the truck onto the ground.

29 CFR  1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye:  (a) Facility, 1050 Ohio Avenue, Glassport, PA:  On or about June 24, 2021 an employee sustained a work-related injury that resulted in a hospitalization and the employer failed to report the incident to the local area office as required.
