OSH ACT of 1970 Section (5)(a)(1):The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to electrical shock hazards. a. On or about September 9, 2021, a Magnetic Lifting Technologies US (MLTUS) electrically-operated magnetic lifting device (Serial number 13016) was used by employees to move scrap material with a Link-Belt Material Handler, model 600LX. The insulation of the electrical wiring to the magnet was damaged to the bare conductors, exposing employees to an electrical shock hazard up to 230 volts.

One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc. a. On or before September 9, 2021, the employer did not ensure employees were protected from coming into contact with the rotating conveyor belt and roller of Conveyor 1 in Building 38. The existing guard did not adequately protect employees from coming into contact with the rotating conveyor belt and roller, thereby exposing employees to caught-in hazards. b. On or before September 9, 2021, the employer did not ensure employees were protected from coming into contact with the rotating conveyor belt and roller of Conveyor 2 in Building 38. The existing guard did not adequately protect employees from coming into contact with the rotating conveyor belt and roller, thereby exposing employees to caught-in hazards.

On or before September 9, 2021, the employer did not ensure that the conductors that entered boxes were protected from abrasion that the openings were effectively closed. The conductors entering the box of the Sun-Mite Infrared heater were exposed to the edge of the box where the connector was loose and pulled away from the box, thereby exposing employees to a 480 volt electrical contact hazard.
