29 CFR  1910.22(a)(3):29 CFR 1910.22(a)(3): The employer did not ensure that walking-working surfaces are maintained free of hazards such as sharp or protruding objects, loose boards, corrosion, leaks, spills, snow, and ice.  On or about September 9, 2021, and at times prior thereto, electrical cords were on the ground in walk ways where employees were performing normal work tasks, exposing employees to a trip and fall hazard.

29 CFR  1910.303(b)(8)(i):Electrical equipment was not firmly secured to the surface on which it was mounted.  On or about September 9, 2021, and at times prior thereto, a wired electrical receptacle was not mounted to a surface but instead hanging against a wooden dresser and it was held in place only by the electrical cord that was plugged into it, exposing employees to an electrical hazard.

29 CFR  1910.305(j)(2)(i):Receptacles, cord connectors and attachment plugs were not constructed so that they would not accept an attachment plug with a different voltage or current rating than that for which the device was intended.  On or September 9, 2021, and at times prior thereto, an electric receptacle that was providing power to a microwave was being powered by an extension cord, exposing employees to an electric hazard.

29 CFR  1910.305(b)(2)(i):Pull boxes, junction boxes, and fittings were not provided with covers approved for the purpose.  On or about September 9, 2021, and at times prior thereto, employees were working in and around areas that had several 120V electrical receptacles and a junction box with missing face plates, exposing the employees to a electrical shock hazard:  a) Three (3) gang receptacle box in the screening area with a two (2) gang cover.  b) Electrical outlet cover missing near the Sportsman EX machine.  C) Electrical junction box missing cover near the exhaust fan.
