29 CFR 1926.501(a)(2):The employer shall determine if the walking/working surfaces on which its employees are to work have the strength and structural integrity to support employees safely. Employees shall be allowed to work on those surfaces only when the surfaces have the requisite strength and structural integrity. On or about June 2, 2021, during the installation of HVAC Equipment at a job site located at 11845 Mahoning Ave, North Jackson, Ohio 44451; the employer did not ensure that the walking/working surface (drop ceiling) would support the weight of the employees working up in ceiling. Employees exposed to a fall approximately 16.5 feet to the concrete floor below.

29 CFR 1926.503(a)(2):The employer shall assure that each employee has been trained, as necessary, by a competent person qualified in the following areas: On or about June 2, 2021, during the installation of HVAC Equipment at a job site located at 11845 Mahoning Ave, North Jackson, Ohio 44451; The employer did not ensure employees working on a walking/working surface were protected from a fall of approximately 16.5 feet by utilizing a fall protection system by providing fall protection training or fall hazard recognition.
