29 CFR 1904.4(a): The employer did not record each work-related fatality, injury or illness case that resulted in the general recording criteria on the OSHA Form 300 or equivalent: Facility at 100 Twin River Road in Lincoln, RI: On or about September 13, 2021, the employer did not record the following work-related injuries or illnesses on the OSHA Form 300 or equivalent: a) Employee A was exposed at work to COVID-19 on August 30, 2021 and tested COVID-19 positive and lost 11 work days due to a work-related illness. b) Employee B was exposed at work to COVID-19 on August 30, 2021 and tested COVID-19 positive and lost 21 work days due to a work-related illness. Abatement Certification and Documentation is required.
