29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking / working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems: a. At the job-site located at 1005 Harbor Lake Drive, Safety Harbor, Florida: On or about September 10, 2021, employees were exposed to fall hazards of more than 20-feet while working on the roof of a systems-engineered metal building, performing roof decking removal and installation. The roof had a 1:12 pitch; 20-feet to the eave and 25-feet to the center top. There were no guardrails, safety net, positioning devices, fall restraint systems or personal fall arrest system.

29 CFR 1926.761(b): The employer did not train each employee exposed to a fall hazard in accordance with the requirements of 29 CFR 1926.761: a. At the job-site located at 1005 Harbor Lake Drive, Safety Harbor, Florida: On or about September 10, 2021, employees were exposed to fall hazards of more than 20-feet while working on the roof of a systems-engineered metal building, performing roof decking removal and installation. Employees were not instructed in the recognition and identification of fall hazards in the work area, the procedures to be followed to prevent falls to lower levels, through or into holes and openings in walking/working surfaces and the fall protection requirements for steel erection.
