29 CFR  1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:   (a) 7991 W. 71st Ave, Arvada CO 80004: On or about September 17, 2021, and at times prior, employees were not provided PPE (e.g. eye protection, hand protection, and clothing/splash protection apron) in the Aspen Neighborhood's oxygen room before transferring liquid oxygen from storage tanks to resident's oxygen tanks. The oxygen transfer room in the Aspen neighborhood did not have appropriate PPE available to employees for exposure to potential leaks from LOX tanks.  Abatement Note: Abatement certification is required for this item (see enclosed Certification of Corrective Action Worksheet).
