29 CFR 1926.501(b)(4)(i):Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around. On or about September 02, 2021 at 6 Shaws Cove, New London, CT job site: The employer did not protect each employee from falling through holes (Skylights) on walking/working surfaces. The skylight opening was 49 inches by 78 inches. Employees were not protected by a standard guardrail, cover, or fall arrest system. The unprotected skylight opening exposed employees to an elevated fall hazard of approximately 9 feet.

29 CFR 1926.502(h)(1)(v):The safety monitor had other responsibilities which could take the monitor's attention from the monitoring function: On or about September 2, 2021 at 6 Shaws Cove, New London, CT job site: The employer did not ensure that the safety monitor was not performing other tasks which could take their attention from the monitoring function. The monitor was observed performing roofing work while other workers were performing work on and near the leading edge of the roof exposing them to fall hazards in excess of 90 ft.
