29 CFR 1926.501(b)(13):  Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):  a) 3477 Crystal Lane, Lancaster, PA, Worksite, House Roof - On or about September 10, 2021,  employees who were performing residential roofing work were exposed to a fall hazard of 17-feet and were not utilizing acceptable means or methods of fall protection.   b) 3477 Crystal Lane, Lancaster, PA, Worksite, Garage Roof - On or about September 10, 2021,  employees who were performing residential roofing work were exposed to a fall hazard of 8.75-feet and were not utilizing acceptable means or methods of fall protection.   Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a.) 3477 Crystal Lane, Lancaster, PA, Worksite, House Roof - On or about September 10, 2021, employees engaged in roofing activities were exposed to a fall hazard while utilizing an extension ladder to access the roof that was only extended 1.5-feet above the roofline and was not otherwise secured.    b.) 3477 Crystal Lane, Lancaster, PA, Worksite, Garage Roof - On or about September 10, 2021, employees engaged in roofing activities were exposed to a fall hazard while utilizing an extension ladder to access the roof that was only extended 2.5-feet above the roofline and was not otherwise secured.    Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR  1926.150(a)(1): The employer did not develop and maintain a fire protection program to be followed throughout all phases of construction and demolition work, and/or did not provide for the firefighting equipment as specified in this subpart:  a.) 3477 Crystal Lane, Lancaster, PA, Worksite, Roof - On or about September 10, 2021, the employer did not have a fire protection plan to ensure fire extinguishers were on-site while employees were operating equipment including, but not limited to, a gasoline-powered air compressor.   Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.
