OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall hazards relating to the use of a material box as a personnel platform on a Skytrack telehandler.   a) 506 West Onondaga Street, Syracuse, NY  13204, on or about 8/31/21:  Employees were accessing and egressing a 9:12 pitch roof with a ground to eave height of 25 feet, from a material box used as a personnel platform, that  was elevated to and from the roof surface.    b) 506 West Onondaga Street, Syracuse, NY  13204, on or about 8/31/21:  Employees were working from a material box used as a personnel platform, that was elevated to the eave of the roof, 25 feet from ground.    The box was not designed as a personnel platform.  It did not have guardrails.  Workers were not provided fall protection.
