29 CFR  1910.28(b)(11)(ii): The employer did not ensure that each flight of stairs having at least 3 treads and at least 4 risers is equipped with stair rail systems and handrails as described in 29 CFR 1910.28 Table D-2-Stairway Handrail Requirements:  a) Ram Fabricating LLC, 412 Wavel Street, Syracuse, NY. Stair way to the Conference Room. On or about 2/28/22,  a fifteen-riser stair way had eleven open sided risers not equipped with handrails on the open side.  Abatement certification must be submitted for the this item

29 CFR  1910.132(d)(1)(i): The employer did not select and have each affected employee use, the types of personal protective equipment that would protect the affected employee(s) from the hazards identified in the hazard assessment:    a) Ram Fabricating, LLC, 412 Wavel Street, Syracuse, NY,  Wash area.  On or about 9/14/ 21, the employer did not select and require employees to use splash goggles and face shields when working at a vat containing undiluted Shineline Floor Prep containing 5-10 % phosphoric acid with a pH < 1. The employees work approximately 6 inches from the vat submerging parts contained in a basket into the vat by the use of a hoist. Employees pour pails of undiluted Shineline Floor Prep containing 5-10 % phosphoric acid with a pH < 1 directly into the vat every 3 weeks.  Abatement certification must be submitted for this item.
