29 CFR 1910.212(a)(1):Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc. a. On or about September 1, 2021, the employer did not ensure that an employee operating the Monarch lathe, machine number 082, was protected from being caught-in and/or caught-around a small approximately three-inch rotating part while polishing the part with a strip of emery cloth. b. On or about May 5, 2021, the employer did not ensure that an employee operating the Monarch lathe, machine number 085, was protected from being caught-around and/or struck-by an unguarded rotating four-jaw chuck while facing steel plates.

Located at Ariel Corporation in Mount Vernon, Ohio: a. On or about April 20, 2021, a work-related injury occurred at the workplace. The employer did not ensure that the object which directly injured the employee was recorded in Column F of the 2021 OSHA 300 Log. b. On or about May 3, 2021, a work-related injury occurred at the workplace. The employer did not ensure that the object which directly injured the employee was recorded in Column F of the 2021 OSHA 300 Log. c. On or about May 24, 2021, a work-related injury occurred at the workplace. The employer did not ensure that the object which directly injured the employee was recorded in Column F of the 2021 OSHA 300 Log. d. On or about June 13, 2021, a work-related injury occurred at the workplace.
