29 CFR  1926.1053(b)(6): Ladders were used on surfaces which were not stable and level:  (a) South side of building @ 11 Willow St. Newport, RI 02840:  On or about 9/10/21, employees were exposed to fall hazards when using a forty foot aluminum extension ladder that was placed in the back of a flat bed truck to gain access to the work area on the roof.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a) South side of building @ 11 Willow St. Newport, Rhode Island 02840:  On or about 9/10/21, employees were using a forty foot aluminum extension ladder that did not extend at least 3 feet above the roof access point.  Slate & Copper Services LLC.  was previously cited for a violation of this occupational safety and health standard, 29 CFR 1926.1053 (b)(1), which was contained in OSHA inspection number 1246284, citation number 1, item number 2, and was affirmed as a final order on 10/30/2017, with respect to a workplace located at 23 Willow Ave. North Hampton, NH 03862.
