29 CFR 1926.502(b)(2): Midrails, screens, mesh, intermediate vertical members, or equivalent intermediate structure members were not installed between the top edge of the guardrail system and the walking/working surface when there was no wall or parapet wall at least 21 inches (53 cm) high. a) 1300 Stedman St. - On or about July 21, 2021, subcontracted employees of Ketchikan Mechanical, Inc. performed ductwork installation in the fan room near a window opening with an unprotected edge while a top rail was installed, a mid-rail, or equivalent was not installed. This condition exposed employees to fall hazards of greater than six (6) feet to the grade below.
