29 CFR 1910.178(n)(1): All traffic regulations shall be observed. The truck shall be kept under control at all times. a.) 4875 N Susquehanna Trail, York, PA - Grocery Warehouse - an employee operating an electric motorized Crown PE4500 pallet jack advanced down a ramp into the Re-Pack material storage area, failed to stop at a posted stop sign, entered into an immediate left turn, and lost control of the pallet jack striking an adjacent storage rack. The employer did not ensure employees operating powered industrial trucks obeyed all warehouse traffic signs/signals and maintained control of the powered industrial trucks during operations, there by exposing employees to injuries, on or about September 8, 2021. b.) 4875 N Susquehanna Trail, York, PA - Grocery Warehouse - an employee operating an electric motorized pallet jack advanced through, and failed to stop at, designated plant stop signs. The employer did not ensure employees operating powered industrial trucks obeyed all warehouse traffic signs/signals during operations, there by exposing employees to injuries, on or about September 15, 2021. Abatement certification required within 15 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.

29 CFR 1910.178(l)(4)(ii)(A): Refresher training in relevant topics was not provided to the operator that has been observed to operate the vehicle in an unsafe manner: a) 4875 N Susquehanna Trail, York, PA - Grocery Warehouse - an employee operating an electric motorized pallet jack advanced through, and failed to stop at, designated plant stop signs. The employer did not take corrective action by providing refresher training upon making observations of unsafe powered industrial truck operations including, but not limited to, failure to obey warehouse traffic signs/signals during operations, there by exposing employees to injuries, on or about September 15, 2021. Abatement certification required within 15 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.
