OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to struck-by hazards: (a) On or about 9/16/2021, and at times prior, 1309 Industrial Blvd, Boaz, AL 35957, Zone 8 and 18, employees working in the warehouse area were exposed to struck-by hazards because several racking system columns had been damaged in an area where PIT operators navigate the aisle to sort and store material.

OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to struck-by hazards: (a) On or about 9/16/2021, and at times prior, 1309 Industrial Blvd, Boaz, AL 35957, restroom at northwest corner of Zone 8 and 18, employees using the restroom were exposed to struck-by hazards from a structurally compromised cinderblock restroom wall that was approximately 10.5'x8' which weighed approximately 3,360 lbs because the employer did not prohibit use of this restroom.
