29 CFR 1910.178(l)(4)(i):Refresher training including an evaluation of the effectiveness of the training, had no been conducted as required by paragraph (l)(4)(ii) to ensure that the operator had the knowledge and skills needed to operate the powered industrial truck safely: On or about August 31, 2021, the employer failed to ensure an employee that had an accident involving a powered industrial vehicle was not retrained. A handler fall off of the forks of the powered industrial forklift when they got caught by "studs" in the jib platform.
