29 CFR 1904.7(b)(3):  The employer did not enter the number of calendar days away from work in the number of days column,  or estimate the number of days that the employee would be away, and update the day count when the actual number of days is known, if the employee would be out for an extended period of time:  A.)  Penn-Mar Castings, LLC, 500 Broadway, Hanover, PA  17331, on or about September 3, 2021, a total number of days away, or an estimate of the number of days away, was not recorded in column K for Case #6 on the 2021 OSHA Form 300 Injury and Illness Log for an incident that occurred on August 27, 2021.  ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below).  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

29 CFR 1904.29(b)(1):  The employer did not enter each recordable injury or illness on the OSHA 300 log, or equivalent form, in the detail as required by the regulation:  A.)  Penn-Mar Castings, LLC, Inc., 500 Broadway, Hanover, PA  17331, on or about June 24, 2021, column F on the 2021 OSHA Form 300 Injury and Illness Log for Case #4, which occurred on June 17, 2021, provided the part of the body affected, but did not include details of the injury or the object/substance that directly injured the person.  B.)  Penn-Mar Castings, LLC, 500 Broadway, Hanover, PA  17331, on or about September 1, 2021, column F on the 2021 OSHA Form 300 Injury and Illness Log for Case #5, which occurred on August 25, 2021, provided the part of the body affected and the injury, but did not include details of the object/substance that directly injured the person.  C.)  Penn-Mar Castings, LLC, 500 Broadway, Hanover, PA  17331, on or about September 3, 2021, column F on the 2021 OSHA Form 300 Injury and Illness Log for Case #6, which occurred on August 27, 2021, provided the injury and part of the body affected, but did not include the object/substance that directly injured the person. 29 CFR 1904.39(a)(2):  The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye.  A.) Penn-Mar Castings, LLC, 500 Broadway, Hanover, PA  17331, on or about August 29, 2021, an employee was hospitalized for surgery for a fractures to the left foot from an accident that occurred on August 27, 2021, and the employer did not report the hospitalization until August 31, 2021.  ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.
