ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

Penn-Mar Castings, LLC, Inc., 500 Broadway, Hanover, PA 17331, on or about June 24, 2021, column F on the 2021 OSHA Form 300 Injury and Illness Log for Case #4, which occurred on June 17, 2021, provided the part of the body affected, but did not include details of the injury or the object/substance that directly injured the person. B.) Penn-Mar Castings, LLC, 500 Broadway, Hanover, PA 17331, on or about September 1, 2021, column F on the 2021 OSHA Form 300 Injury and Illness Log for Case #5, which occurred on August 25, 2021, provided the part of the body affected and the injury, but did not include details of the object/substance that directly injured the person. C.) Penn-Mar Castings, LLC, 500 Broadway, Hanover, PA 17331, on or about September 3, 2021, column F on the 2021 OSHA Form 300 Injury and Illness Log for Case #6, which occurred on August 27, 2021, provided the injury and part of the body affected, but did not include the object/substance that directly injured the person. Penn-Mar Castings, LLC, 500 Broadway, Hanover, PA 17331, on or about August 29, 2021, an employee was hospitalized for surgery for a fractures to the left foot from an accident that occurred on August 27, 2021, and the employer did not report the hospitalization until August 31, 2021.
