29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii): a) Jobsite located at 1330-1334 South Patton Street, Philadelphia, PA - The employer did not ensure that the scaffold was fully planked. Employees were building an elevator tower and working from the top level of the scaffold; thus exposing the employees to trip and fall hazards of up to 40 feet. Condition was observed on or about September 13, 2021.

The employees had to access and egress the scaffold by climbing on the scaffold frames and crossbraces; thus exposing employee to fall hazards. Condition was observed on or about September 13, 2021.
