29 CFR 1926.20(b)(1): The employer did not initiate and maintain an accident prevention program as may be necessary to comply with this part.  a) Jobsite located at 914 South Avenue, Secane, PA - The employer did not develop and implement an accident prevention program to assist in identifying working conditions which are hazardous to the health or safety of his employees, such as, but not limited to, fall hazards.  Condition noted on or about September 13, 2021.

29 CFR  1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b):  a) Jobsite located at 914 South Avenue, Secane, PA - The employer did not ensure that employees were protected from falls. Employees were working from a residential roof greater than 6 feet in height without the use of a fall protection system; thus exposing the employees to fall hazards.  Condition was observed on or about September 13, 2021.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a) Jobsite located at 914 South Avenue, Secane, PA - The employer did not provide fall protection training to employees engaged in roofing activities on a residential building approximately 25 feet above the ground level. The employees were not trained on how to recognize, evaluate, and mitigate hazards.  Condition was observed on or about September 13, 2021.
