29 CFR  1926.102(a)(1):The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  On or about September 07, 2021, at a construction site in Katy, TX, employees were using pneumatic nailers to install roof decking without eye protection.

29 CFR  1926.501(b)(1):"Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  On or about September 7, 2021, at a construction site in Katy, TX, employees were working on a roof, installing roof decking, more than 6 feet above the ground without the use of a fall protection system.

29 CFR  1926.1051(a):A stairway or ladder shall be provided at all personnel points of access where there is a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist is provided.  On or about September 07, 2021, at a construction site in Katy, TX, the employer did not provide a ladder to the employees who were working on the roof.
