29 CFR 1926.453(b)(2)(v):A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift. On or about September 2, 2021, on the east side of building located at 107 Old River Road in Montgomery, TX, employees were exposed to fall hazards while accessing steel beams on a structure from a JLG Aerial Lift basket without fall protection system.

29 CFR 1926.760(a)(1):Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems. On or about September 2, 2021, at a construction site located at 107 Old River Road in Montgomery, Texas, an employee was exposed to fall hazards exceeding more than 15 feet above a lower level while welding from the second level floor without a fall protection system in place.
