29 CFR  1926.100(a):Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns.  On or about September 16, 2021, and at times prior thereto, employees were engaged in plumbing installation on a three story construction site. The employees were working below other construction workers that were using hand tools and other construction material with out the use of a hard hat, exposing them to a struck by hazards by tools or materials falling from a higher level.

29 CFR  1926.453(b)(2)(v):A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift.  On or about September 16, 2021, and at times prior thereto, an employee was on a aerial lift at heights above 6 feet installing a floor drain to the interior of a commercial building with out the use of fall protection, exposing the employee to a fall hazard.

29 CFR  1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  On or about September 16, 2021, and times prior thereto, employees were doing work on on a 3 story commercial building with out the use of fall protection on levels two and three, exposing the employees to the hazard of falling approximately 15-25 feet to a lower level.

29 CFR  1926.502(d)(21):Personal fall arrest systems were not inspected prior to each use for wear, damage, and other deterioration, and/or defective components were not removed from service:  On or about September 16, 2021, and at times prior thereto, the employer provided his employees harnesses and lanyards for fall protection, but they were not inspected daily before use, exposing the employees to a fall hazard.
