On or about September 20, 2021, during the restoration of the barn doors located at 294 Pearl Rd, Brunswick, OH 44212, the employer did not ensure an employee working from a JLG Model 660JP aerial lift had the ability to recognize and avoid unsafe conditions. The employee was working inside the aerial lift basket, but could not operate the controls to maneuver the aerial lift.  The employee did not have the ability to move the aerial lift basket in any direction.
