29 CFR 1910.119(e)(3)(i): The Process Hazard Analysis did not address the hazards of the process a) The PHA did not address all the hazards of the process. Vantage did not fully consider, in the August 2021 PHA, potential impacts from malfunctions or explosion events in the boilers on the structural supports, pipes, ductwork, roofing and equipment associates with the process containing HHC. In accordance with 29 CFR 1903.19 (d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstration that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of purchase or repair of equipment, photographic or video evidence of abatement or other written records.
