29 CFR  1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  On or about September 20, 2021, at a worksite located at Lot 17 Cardinal Meadow Ct., Mentor, OH 44060, the employer did not ensure employees were protected from possible head injuries by ensuring head protection was worn during exposure.  An employee was working below an employee cutting vinyl siding and handing/tossing up material to an employee on a ladder jack scaffold. The employee on the ground was not wearing any head protection.

29 CFR  1926.454(a):  The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards:  On or about September 20, 2021, the employer did not have a training program established for work on scaffolding and employees failed to understand the hazards and procedures to minimize the hazards.  Employees were exposed to a fall of 17.2 feet, but planned to go higher and employer was unaware of height restriction for ladder jack scaffolding.

29 CFR  1926.451(g)(1)(i):  Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:  On or about September 20, 2021, at a worksite located at Lot 17 Cardinal Meadow Ct., Mentor, OH 44060, employees were exposed to fall hazards of approximately 17.2 feet while working from a ladder jack scaffold. Employees were installing vinyl siding material with no fall protection.  Alberto Davilla Siding was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.451(g)(1)(vii), which was contained in OSHA inspection number 1262143, citation number 01, item number 02 and was affirmed as a final order on November 24, 2017, with respect to a workplace located at S/L 79 Corbetts Lane, Twinsburg, OH 44087.
