1910.147(a)(1)(i)  This standard covers the servicing and maintenance of machines and equipment in which the energization or start up of the machines or equipment, or release of stored energy, could harm employees. This standard establishes minimum performance requirements for the control of such hazardous energy.   Employer is failing to ensure that employees affix lockout devices during servicing of equipment. On or about September 7, 2021, two employees reinstalling the conveyor belt on the Trim Blend metal detector failed to control energy by installing lockout devices on the disconnect switch. One employee turned the conveyor belt on, crushing the right hand fingers of the other employee in the belt drive sprocket(s).
