Flexible cords shall be connected to devices and fittings so that strain relief is provided which will prevent pull from being directly transmitted to joints or terminal screws. (a) Location - 157-159 Agawam Street, Lowell, MA: On or about 9/22/2021, employees were exposed to electrical shock hazards while using a flexible electrical cord that had the outer sheathing pulled away from the strain relief device, exposing the insulated conductors.
