At the jobsite, employees operating Hitachi nail guns while performing framing activities were not wearing eye protection, exposing employees to the hazard of being struck by flying objects.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system.    At the jobsite, employees engaged in framing activities at a height of over 6 feet were not protected by a fall arrest system. Employees were exposed to an unprotected fall of over 8 feet to the ground below.

29 CFR  1926.405(b)(2): Covers and canopies. All pull boxes, junction boxes, and fittings shall be provided with covers. If metal covers are used, they shall be grounded. In energized installations each outlet box shall have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear.    At the construction site, the electrical receptacle box was not equipped with a cover plate, exposing employees to the hazard of electrical shock from the internal energized parts.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  At the jobsite, employees performing framing activities were not provided fall protection.  The employer did not provide a training program with hazard recognition and avoidance training where part of the job is to be exposed to fall hazards at a heights over 6 feet to the ground below.
