The employee was wearing a pair of company provided gloves as he placed rubber pieces into the rubber mill. The employer did not verify that the required workplace hazard assessment had been performed through a written certification that identifies the workplace evaluated, the person certifying that the evaluation had been performed and the date of the hazard assessment, on or about 9/23/21
