29 CFR  1910.178(m)(5)(ii):A powered industrial truck shall not be left unattended when the operator is 25 ft. or more away from the vehicle which remains in view, or whenever the operator leaves the vehicle and it is not in view.  At jobsite located at 801 S. Meridian Valley Center, KS, employees engaged in commercial roofing activities, were exposed to struck-by, crushing hazards in that the JLG Telehandler was left unattended while the operator was more than 25 feet away.

Meridian Valley Center, KS, employees engaged in commercial roofing activities were exposed to electrical and fire hazards in that the splices were not covered with insulation on the extension cord that was powering equipment at the site.
