On or about September 22, 2021, at the jobsite, the employer did not ensure employees were in use of eye protection while using pneumatic nail guns and hammers to install roofing materials to the roof of a single-family single-story building. This exposed the employees to struck-by hazards.

On or about September 22, 2021, at the jobsite, the employer did not ensure employees were in use of fall protection while installing roofing materials to the roof of a single story multi-family residential building. This exposed the employees to a fall hazard greater than six(6) feet to the ground below.
