On September 23, 2021 at a worksite located 343 Bell Street, Neenah, WI 54956; the employer did not provide for frequent and regular inspections of the job site and equipment used. Employees were exposed to fall hazards while working on a residential roof without fall protection and were exposed to eye hazards from flying particulate during pneumatic nailing without eye protection.

Three employees installing roofing shingles on a residential building were exposed to falls from the roof approximately 9 feet above the lower level.

The program shall enable each employee to recognize hazards related to ladders and stairways, and shall train each employee in the procedures to be followed to minimize these hazards. 29 CFR 1926.501(b)(13): Residential construction. Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. Exception: When the employer can demonstrate that it is infeasible or creates a greater hazard to use these systems, the employer shall develop and implement a fall protection plan which meets the requirements of paragraph (k) of 1926.502: On September 23, 2021 at a worksite located 343 Bell Street, Neenah, WI 54956; employees engaged in roofing activities on a residential home were working approximately 9 feet above the lower ground level and were not protected by guardrail systems, safety net systems or personal fall arrest systems.
