Location: 5017 S 38the Street, Saint Louis, Missouri, Extrusion Line Nos. 9 and 10.  a) On or about September 22, 2021, extrusion line No. 9, the knife gate was not fully guarded exposing the operator to finger(s) amputation. The Knife gate measuring 8 inches wide and 4 inches high was wide enough to allow the fingers to reach in to the knife (Guillotine Cutter).   b) On or about September 22, 2021, extrusion line No. 10, the knife gate was not fully guarded exposing the operator to figure amputation. The knife gate measuring, 7 inches wide and 4.5 inches high allowed the operator's fingers to reach the knife (Guillotine cutter).
