29 CFR  1926.102(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  (a) L & G Roofing Services, LLC located at 2800 Arapahoe Rd. Erie, CO 80516;  On 9/8/2021 and at times prior, the employer did not ensure that eye protection was used when employees were exposed to eye injury while using pneumatic nail guns.  Employees were installing shingles on the roof of the clubhouse with a pneumatic nail gun without using eye protection. This condition exposed employees to struck by hazards.

29 CFR  1926.501(b)(1): Each employee on a walking/working surface having an unprotected side or edge which was six or more feet (1.8m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest system:    (a) L&G Roofing Services, LLC located at 2800 Arapahoe Rd. Erie, CO 80516; on 9/8/2021 and at times prior, the employer did not ensure that employees installing shingles on a roof were protected by guardrail system, safety net systems, or personal fall arrest systems at all times when they were exposed to falls greater than 6 feet.  One employee was exposed to falls exceeding 8 feet when working on the roof of the clubhouse installing shingles without using any form of fall protection.  This condition exposed employees to fall hazards.

29 CFR  1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards:  (a) L & G Roofing Services, LLC located at 2800 Arapahoe Rd. Erie, CO ; On 9/8/2021 and at times prior, the employer failed to provide training to each employee exposed to a fall hazard.  Two employees were installing shingles on the roof of the clubhouse without receiving training  on the nature of fall hazards in the work area and how to minimize fall hazards.  This condition exposed employees to fall hazards.
