The employer did not ensure that the energy control procedure for entering grain storage bins were inspected annually.

29 CFR 1910.147(c)(7)(i):The employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees: Heritage Cooperative, Inc. doing business as Heritage Cooperative LLC. located at 13118 Ohio 31 in Kenton, Ohio 43326: On or about September 1, 2021 the employer did not ensure that employees who engage in group lock out tag out activities were adequately trained in the skills required for the safe application, usage, and removal of energy controls. Employees were exposed to engulfment and entrapment hazards in that each employee engaged in cleaning activities inside of grain bins number #9 and #5 did not apply a personal lock out tag out device.
