29 CFR 1926.1203(g)(3): The entry employer did not document the basis for determining that all hazards in a permit space have been eliminated or isolated, through a certification that contains the date, the location of the space, and the signature of the person making the determination. The certification must be made available to each employee entering the space or to that employee's authorized representative: (a) East Providence High School: On or about 9/23/2021, the entry employer did not document the basis for determining that all hazards in a permit space including surface water storm water collection manholes, have been eliminated or isolated, through a certification that contains the date, the location of the space, and the signature of the person making the determination.

29 CFR 1926.1207(d): The employer did not maintain training records to show that the training required by paragraphs (a) through (c) of this standard has been accomplished. The training records must contain each employee's name, the name of the trainers, and the dates of training. The documentation must be available for inspection by employees and their authorized representatives, for the period of time the employee is employed by that employer. (a) East Providence High School: On or about 9/23/2021, the employer did not maintain training records to show that the training required by paragraphs (a) through (c) of this standard for entry supervisors, attendants and authorized entrants had been accomplished for employees involved with entering storm water manholes.
