29 CFR 1926.451(c)(2)(v): Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork and the fork-lift was moved horizontally while the platform was occupied. On or about September 27, 2021, on the south side of the site, employees were standing/riding on a shop-built platform that was not secured to the forks of an all-terrain forklift.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section. On or about September 27, 2021, on the Southside of the site, an employee was utilizing a shop-made basket at a height greater than ten (10) feet, performing outer wall insulation installment without proper fall protection or appropriate guardrail system.
