29 CFR  1926.652(a)(1): Each employee in an excavation shall be protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section:  On or about September 28, 2021 at a jobsite located at the end of Steffens Court in Green Bay, Wisconsin; an employee was exposed to cave-in hazards while installing a new 18-inch PVC Gravity Sewer Pipe in an approximate 17 feet 5 inch deep trench.  The double stacked trench boxes did not extend at least 18 inches above the top of the vertical side, nor were the sides of the trench sloped and/or benched one-to-one in accordance with Type B Soil Requirements.
